Community Health Improvement Plan
The Greenup County Health Improvement Plan (CHIP) is a roadmap for improving the health and quality of life of everyone in our county. It builds on the findings from our Community Health Assessment and focuses on the priority health issues that matter most to our community members. The priority health issues identified were: Dental Care, Diabetes, Overweight/Obesity, Tobacco Cessation, and Access to Care (transportation). The CHIP outlines specific goals, strategies, and collaborative actions we will take over the next several years to address these priorities. It emphasizes collaboration and accountability, ensuring that progress is measured and shared with the public. By working hand in hand with community members, organizations, and local leaders, we’re aligning our efforts and resources to create lasting, possible change and to build a healthier, more connected community for all.
