Preventative Programs

We have a variety of preventative health programs for children and adults which ensure the ongoing good health of everyone in our community. The Health Department is a source of information and services for individuals and families who need assistance with medical care, nutritional counseling, health screenings, immunizations, family planning, environmental protection and disaster preparedness.

Community Health Assessment (CHA)

A Community Health Assessment (CHA) is a comprehensive process that gathers and analyzes data on the health status, behaviors, and needs of a community. It helps identify key health issues and factors that influence well-being, such as social, economic, and environmental conditions. The findings collected from community voices and health data help guide community partners and our Greenup County Health Department in setting priorities, developing strategies, and allocating resources to improve overall community health.

Community Health Improvement Plan (CHIP)

A Community Health Improvement Plan (CHIP) is a collaborative, long-term plan that outlines strategies to address the priority health needs identified in the Community Health Assessment (CHA). It brings together community partners, stakeholders, and community members to set measurable goals and implement evidence-based actions.
